This is the current process from the Texas Department of State Health Services. Step-by-Step Guide. How to Request a Vaccine Exemption Form Step 1. Gather Your Information - Full name and date of birth of each student
- Parent or guardian’s mailing address
- Number of forms needed (maximum of 5)
Step 2. Request the Exemption Form You have three options: - Online:Request the form here
- Mail a written request to: Texas Department of State Health Services, Immunization Section, Mail Code 1946, P.O. Box 149347, Austin, Texas 78714 – 9347
- In person: 1100 West 49th Street, Austin, Texas 78756
Step 3. Notarize the Form - Do not sign the form until you are in front of a notary.
- All exemption forms must be notarized.
Step 4. Submit the Completed Form - Turn in the form to your child’s school within 90 days of being notarized.
- Exemption forms are valid for two years.
