How to Request a Vaccine Exemption Form!

Texans for Medical Freedom is working hard to ensure every Texas parent has the tools they need to protect their child’s medical freedom.

If your child needs a vaccine exemption for school, follow our step-by-step guide to find out how to submit the form before school starts. This is the current process from the Texas Department of State Health Services.

Step-by-Step Guide. How to Request a Vaccine Exemption Form

Step 1. Gather Your Information

• Full name and date of birth of each student
• Parent or guardian’s mailing address
• Number of forms needed (maximum of 5)

Step 2. Request the Exemption Form

You have three options:

• Online:Request the form here
• Mail a written request to: Texas Department of State Health Services, Immunization Section, Mail Code 1946, P.O. Box 149347, Austin, Texas 78714 – 9347
• In person: 1100 West 49th Street, Austin, Texas 78756

Step 3. Notarize the Form

• Do not sign the form until you are in front of a notary.
• All exemption forms must be notarized.

Step 4. Submit the Completed Form

• Turn in the form to your child’s school within 90 days of being notarized.
• Exemption forms are valid for two years.
